In the winter of 1854–55, the British Army was losing a war against its own hospitals. At the Barrack Hospital at Scutari, across the water from Constantinople, soldiers evacuated from the Crimean battlefields lay in corridors above blocked sewers, and they died — not mainly of Russian shot, but of cholera, typhus and dysentery. Of the roughly 21,000 British soldiers who died in the Crimean War, around four in five were killed by disease rather than by the enemy. At the hospital's worst, in February 1855, men admitted to Scutari were dying at a rate that, had it continued, would have consumed the entire British Army in the East within a year.
Into this had arrived, in November 1854, a 34-year-old gentlewoman named Florence Nightingale — born in and named after the city of Florence — leading a party of 38 nurses. The newspapers made her a saint; the reality was grimmer and more interesting. Nursing alone barely dented the death rate. What changed everything came in March 1855, when the government's Sanitary Commission reached Scutari and found the hospital sitting on choked sewers, its water supply contaminated — the commissioners' reports describe filth up to and including the carcass of a dead horse fouling a water channel. They flushed the drains, cleared the grounds and opened the wards to air, and the mortality curve fell off a cliff. Nightingale came home in 1856 haunted by a conclusion she could prove: her soldiers had been killed not by wounds or by fate but by bad drains, and by an administration that had never counted the cost.
Turning grief into a graph
What she did next is why statisticians claim her as one of their own. Backed by Queen Victoria's support for a Royal Commission on the health of the army, Nightingale spent the late 1850s buried in mortality returns alongside William Farr, the pioneering medical statistician at the General Register Office. As a woman she could neither sit on the commission nor vote for the MPs who would act on it; her weapon was evidence. Her 1858 report, Notes on Matters Affecting the Health, Efficiency and Hospital Administration of the British Army, ran to well over 800 pages of tables and analysis.
Buried in it was her masterstroke: the Diagram of the Causes of Mortality in the Army in the East, now known as the rose diagram. Two flower-like charts, one for each year of the war, divide the months into wedges radiating from the centre. The area of each wedge shows the deaths that month: blue-grey for preventable zymotic disease, red for wounds, black for everything else. In the first winter the disease wedges balloon monstrously, dwarfing the red of battle; after the sanitary reforms of spring 1855, the whole rose visibly shrinks. The design is often called a coxcomb, though that is a misnomer — Nightingale used the word for the pamphlets, not the charts. The intent, as she explained, was to reach through the eyes those whom written reports would never touch: ministers, generals and the public could take in at a glance what no column of figures would ever make them feel. It stands among the most influential statistical graphics ever drawn, alongside the work of William Playfair, who had invented the pie and bar charts decades earlier.
The lamp and the ledger
The campaign worked. Her data showed that even in peacetime, soldiers in British barracks were dying at nearly double the rate of civilian men the same age; killing men through negligence at that scale, she argued, was as criminal as marching a thousand of them onto Salisbury Plain and shooting them. Barracks were rebuilt, army sanitary administration was overhauled, and military medical statistics were put on a professional footing. Recognition from her true profession followed: in 1858 she became the first woman elected a Fellow of the Statistical Society of London, later the Royal Statistical Society, and in 1874 an honorary member of the American Statistical Association.
Posterity, though, preferred the lamp. A report in The Times had described her night rounds at Scutari, and Longfellow's 1857 poem Santa Filomena fixed the image of the Lady with the Lamp in the world's imagination — a gentle icon of mercy, scrubbed of mathematics. She would have found the edit exasperating. Her 1859 book Notes on Nursing became a bestseller, and the Nightingale Training School she founded at St Thomas' Hospital in 1860 made nursing a profession; but her deeper method was always the ledger, not the lamp — count the dead, find the cause, shame the state into prevention. Honours arrived late: in 1907 she became the first woman appointed to the Order of Merit. She died in 1910, aged 90, and her birthday, 12 May, is still marked as International Nurses Day. The lamp made her famous; the charts made her right — and it was the charts that kept the next army of young men alive.
